You can contact the office by calling or texting to request an appointment. You may also use the form below to include your name, email, and phone number in order to book your appointment.  Once I receive your information, I will contact you to answer any questions or to schedule your appointment within 24 hours. 






What to Expect Next
You will be contacted to confirm the request and to schedule a day and a time that is available. Once the appointment is booked, you will receive two emails. The first email is confirming the appointment date and time. The second email is for you to review my policy and to complete a client intake form prior to your appointment. This will allow me time to review and be prepared to discuss any issues that you may have at the time of the appointment.
